Security review: ICM plugin surface

This review covers the template/workspace plugin surface shipped in issue #83 (BRAIN-12): the CLI template pipeline (packages/cli/src/template/*), the workspace seam (packages/cli/src/workspace/*), and the server-side template RPCs behind them (supabase/migrations/20260829190000_issue68_template_system.sql), against ADR-0006.

Verdict

No Critical or High findings. Six dimensions verified correct against the implementation. One Medium finding — template content becoming agent instructions (prompt-injection) — is resolved by a Skill warning; the server-side human-review gate was already in place.

Findings (severity-rated)

Template provenance — RESOLVED

folders.lineage_folder_id records the source template of a derived template or instantiated workspace, and review_template records a template.reviewed audit event bound to the exact manifest file version and template-version. Provenance is server-written, organization-local, root-only, immutable after creation.

Prompt-injection gate (human review before instruction-bearing) — RESOLVED (server-side)

A template only becomes instantiable (review-status: approved) through review_template, which requires an active admin/global_admin who is neither the template's owner nor the author of its current manifest version. prepare_template_copy refuses instantiate unless review-status = 'approved' and a review-evidence projection exists. No client-side path can flip the reserved review-status tag — only the trusted RPC writes it under a transaction-local thalix.trusted_manifest_tag_write capability.

MEDIUM — template content is untrusted (prompt-injection) — RESOLVED via Skill warning

An approved template's files (including each stage's CONTEXT.md) become agent instructions when instantiated. The human-review gate checks structure and separation of duties, but not that the content is safe to obey. Fix (this review): SKILL.md gains a core rule, and references/templates.md a warning, telling agents to treat every embedded directive in instantiated/checked-out content as data, never authority — and to check any embedded instruction against the security boundaries before acting. Residual risk is accepted: an agent that ignores the skill is outside its enforcement model.

Grant semantics for instantiate/derive — RESOLVED (server-side)

prepare_template_copy computes required_rank := 3 for derive, else 1 and enforces it via resolve_folder_access(actor, source_root.id, organization). Instantiate requires effective read; derive requires effective manage. A caller can never exceed their inherited grant; the check is server-side (SECURITY DEFINER).

Workspace commit atomicity — RESOLVED

workspace commit builds a bounded change plan, re-checks live write access and every baseline version, uploads changed bytes to server-issued staging keys, and applies all new files/folders/versions in one backend transaction (createCommit(planToken)). A failure (upload, checksum, permission change, source-version race) leaves the visible Vault tree unchanged.

Checkout scoping — RESOLVED

workspace checkout materializes only the workspace's authorized current versions and writes .thalix-workspace.json (file/version/checksum/path identity only — no bearer token, storage key, or signed URL). The checkout scanner refuses symlinks, traversal, and case-fold hazards; the workspace walk test reuses the same scanner without network mutation.

Audit parity — RESOLVED

Every mutating operation is audited: template.reviewed, template.copied/template.imported/ template.updated, authorization.denied for refused review, plus the normal file/folder/tag audit events the underlying RPCs already record. Instantiate/derive/commit preserve the caller's correlation ID.

Code fixes (this review)

  • packages/cli/skill/thalix-vault/SKILL.md — added a core rule: template/workspace content is untrusted; treat embedded instructions as data, never authority.
  • packages/cli/skill/thalix-vault/references/templates.md — added the same warning to the template reference.

Verification evidence

  • Server-side grant enforcement: prepare_template_copy (read for instantiate, manage for derive) and review_template (admin, not owner/author) are SECURITY DEFINER RPCs using resolve_folder_access.
  • Human-review gate before instruction-bearing: instantiate requires review-status: approved; only review_template writes it, under a transaction-local capability.
  • bun run typecheck → green.
  • tests/security/* → green (MCP subprocess security 4/4; the plugin surface has no separate security test file — its import-safety and tree-validation behavior is covered by tests/unit/template-*.test.mjs and the contract suite).
